  • Complete list of assets and debts
  • Estimated income and expenses for a full year
  • Most recent statements from all investment accounts
  • Copy of most recent federal income tax return (three years, if self-employed)
  • Employer benefit and self- funded plan information (life and disability insurance, retirement, etc.)
